Our nurse practitioner and advanced practice providers (APP) collaborate with all disciplines of the healthcare team, patients, and their caregivers to provide value-based, patient-centered care.

• Access to Community Resources to Assist with Activities of Daily Living (ADL)
• Address Dietary and Nutritional Concerns
• Coordinate with Family Medicine, Internal Medicine and Other Physicians Providing Patient With Treatment or Care
• Assist in Arranging Follow-up Medical Services Appointments with Specialists through Referrals and Scheduling Assistance:
  • Home Health Care Services for Physical Therapy, Occupational Therapy, Speech Therapy, etc.,
  • Podiatry / Wound Care for In-Home Foot Exams
  • Ophthalmology for In-Home Eye Exams
  • Psychiatry
• Coordinate Radiological Imaging for Standard X-rays, Ultrasounds, and Echocardiograms
• Order Bloodwork and Other Testing Via A Third Party Mobile Healthcare Laboratory.
